survey to be sure your routine immunizations are
up to date.

need to register if this is your first time to use the service.
Your physician will review this information and we will contact you to
arrange the appointment with our nurse to complete the immunizations- please
allow us 48 hours to review and obtain the necessary approvals by your health
plan before scheduling this appointment.
Please be sure to have our staff complete your immunization records with
all your immunizations and keep this with you along with your passport during
your travels.
