Travel Planning at SMG

The SMG web site has several useful tools and links to review and set up travel immunization appointments and WebVisits. Please note that SMG only provides travel advice to established patients. If you are not a current patient, please call us and sign up for an appointment with the physician of your choice.

We recommend you review the following information prior to setting up an appointment at SMG to review your travel health needs:

• CDC Travelers' Health Page . At this site, you can reivew the current recommendations for the countries you are visiting and review up to the minute updates concerning Health Warnings or Precautions for these regions.
  
• U.S. Travel Advisory Site. Americans traveling abroad and foreign citizens traveling to the U.S. will find information here regarding Travel Warnings, access to U.S. Embassies and Consulates while abroad, and help obtaining necessary travel documents.
  
• Vaccination Survey. This brief survey will ensure your routine immunizations are up to date prior to your trip.
  

Setting up an appointment is easy. Please complete the travel Webvisit on our secure site. This information will be emailed to your physician who will review your chart and other health needs and contact you to arrange an appointment.

Please allow us 48 hours to review your information and obtain insurance authorization when needed so we can better serve you. Most patients will require an appointment with our staff, if you have other needs or would like to meet with your physician, please note that on the WebVisit or call us at your convenience.
